Attached please find a copy of your Castlegar Minor Hockey Registration Form for the 2012/2013 Season. Please complete this form in FULL.

Please mail your completed Registration Form to the following:

CMHA Registrar – P.O. Box 3264, Castlegar, BC V1N 3H5 (also listed on the top of the Registration Form).

You may also drop your completed Registration Form off at the Castlegar Complex front desk. (Ask the front end staff to put it in the CMHA Mail Box). PLEASE make sure your Registration Form and payment are in an envelope, Attention to: CMHA REGISTRAR.

All Registrations must have payment attached - in order for the Registration Form to be processed. (If there is no payment submitted, your applications may fall under the “Late Registration” category, and you will be charged the late fees)

IMPORTANT: If you register for the 2012/2013 season PRIOR to May 31^st, 2012 - you will receive a $50 discount! We allow postdated cheques for the season… please see details on the Registration Form.

Lastly, if you are applying for the Kid Sport Grant, or the Jump Start Grant, you MUST SUBMIT

A COPY of your completed application forms attached to your CMHA Registration Form. These forms are your responsibility to submit directly to Jump Start and Kid Sport, but I must have a copy of your application attached to the Registration Form with the DATE you submitted them. (The Late Fee’s will be the responsibility of the player/parent if your CMHA Registration is not in by the May 31^st Deadline!)
